Muscle Transplantation as a Rescue Procedure in Large Residual Strabismus

Purpose: To describe two cases where large angle residual strabismus was corrected by single muscle resection and transplantation of the resected segment to its antagonist.

Methods: Retrospective case series. Case 1–There was residual esotropia of 40 prism diopters (PD) after three horizontal rectus muscle surgery for infantile esotropia. The Lateral rectus muscle (LR) was resected 9mm and transplanted to the already recessed medial rectus muscle (MR). Case 2-There was residual exotropia of 40 PD after 8mm LR recession for sensory exotropia. MR was resected 6.5mm and transplanted to the already recessed LR.

Results: Both the cases showed excellent alignment (within 10PD of orthotropia) at their last follow up of 1 year and 6 months respectively. There was minimal duction limitation (-1) in the field of the already recessed muscle in both cases.

Conclusion: Muscle transplantation procedure can be an effective procedure for correction of residual horizontal deviation.
